Below you will find a sample fee schedule to assist you in your decision and investigative process. While the fees below are subject to change and can vary slightly on the individual case, they represent a good baseline of what the majority of our patients experience.

Global Procedure Fees Include:
Physician Services Refractive procedure.
Post-operative examinations for 12 months following procedure, there are usually 6 post-op exams.
Will charge insurance company after your first annual exam and co-pay is expected.
Facility Services
Outpatient refractive procedure (including enhancements, for that procedure).
Nursing assistance during the procedure.
Pre-operative and intra-operative medications.
Post-operative care.

Payment is required on day of your procedure. All returned checks will be an additional $25.00

Please inquire about our payment options. Some employers offer their employees a Cafeteria Plan benefit that allows you to use pre-tax dollars for medical expenses not covered by insurance. Please check with your employer to see if this option is available where you work.
